We know life gets busy, so we’ve made requesting an appointment quick and easy through our website. Once you submit your request, a member of our team will contact you to confirm your needs and schedule a time that works best for you.

Before your first visit
- Call your insurance company to verify that your chosen provider (our dentist) accepts your insurance and is in your network.
- Download both forms (below), print and fill out the new patient forms and office policy so you can bring them with you to your first appointment.

- Insurance Card
- New patient forms: New Patient Health History and Office Policy Agreement
- As a new patient, our office will verify your insurance before your appointment to confirm that your plan is active and in-network.
As a new patient, we kindly ask that you arrive 10 minutes early for your appointment to complete any necessary paperwork.
Upon arrival, you’ll find our office just off Route 516 - the Green Building next to the car wash. Please drive to the back of the building for convenient parking and entry to office.
